  • To support care coordination and system/service navigation for persons with stroke and their care partners

Developing this Website

This website was developed by the Aging, Community, and Health Research Unit at McMaster University’s School of Nursing with the help of our patient partners, as well as the Patient and Family Advisory Council at the Central South Regional Stroke Network.

Developing the Map

The “My Stroke Recovery Journey Map” was developed by the Central South Regional Stroke Network.

The original concept of a graphic representation for the stroke recovery journey was developed by the Northeastern Ontario Stroke Network. Subsequently the Central East Stroke Network developed their “Journey to Recovery after Stroke” resource, which included a collaborative and comprehensive set of documents including a Clinicians User Guide.

Based upon the foundational work of these two groups, the Central South Stroke Network supported the work of the Central South Regional Stroke Network Patient and Family Advisory Council to develop an aphasia friendly My Stroke Recovery Journey resource inclusive of a Clinicians User Guide. The process was interactive and collaborative with strong patient voice representation. Stakeholder feedback was garnered across the stroke continuum of care and the resource underwent a pilot process prior to finalization.

The Central South Stroke Network would like to acknowledge the work of the Central East Stroke Network, specifically the CESN Journey to Recovery Clinicians’ User Guide, which informed the development of the Central South Stroke Recovery Journey Clinician User Guide.
