INTERPROFESSIONAL BEST PRACTICE WEBINAR SERIES 2025
This year our webinar series will focus on Patients and Families.
Target Audience: These webinars are intended for physicians, nurses, speech-language pathologist, physiotherapists, occupational therapists, dietitians, social workers, and other health care professionals providing stroke care across the continuum.
The first 2 webinars are a two part workshop presented by Sandra Andreychuk, RN, MHSc. (Bioethics), MSc. (Nursing). She is a Clinical Nurse Specialist, Advance Health Care Consultant, and a Health Care Ethicist
Advanced Goal of Care Conversations: Empowering Health Care Professionals in Informed Consent and End-of-Life Planning for the stoke patient population
This two-part workshop is designed to equip health care professionals with the necessary skills and knowledge to confidently engage patients and their families in meaningful discussions around goal of care and end-of-life care planning. Utilizing the principles of informed consent and the Serious Illness Conversation Guide, participants will learn how to facilitate these important conversations with empathy, clarity, and respect for patient autonomy.
WEBINAR 1 – Part 1: Foundations of Goal of Care Conversations

Objectives:
- To introduce the principles of informed consent and its application in goal of care discussions.
- To familiarize participants with identifying the SDM when a patient is found incapable for health care decision making.
- Describe various types of care planning in healthcare settings, commonalities and differences
- Case study review in small break-out rooms
- Sharing experiences and common barriers faced during goal of care conversations.
- Collaborative brainstorming on overcoming challenges.
WEBINAR 2 – Part 2: The Serious Illness Conversation Guide (SICG) framework

Objectives:
- To enhance communication skills necessary for conducting effective end-of-life conversations.
- Discuss triggers to identify patients who would benefit from a serious illness conversation
- Discuss key components of the guide (understanding the patient’s perspective, prognosis, exploring key topic, making recommendations).
- Explored practical application of the SICG using case studies
WEBINAR 3 – The Art of Client Education

This webinar will be presented by Theresa Straathof, OT Reg. (Ont.). She works at The Ottawa Hospital in Acute Mental Health. She has written two manuals related to teaching coping strategies. She has presented nationally and internationally on topics related to behaviour activation, coping strategy training, and adult learning.
Objectives:
- Describe the stages of Kolb’s Adult Learning Model (ALM) (2014)
- Construct a learning session using Kolb’s ALM
- Adapt ALM delivery model for aphasia
WEBINAR 4 – Shifting your Mindset: The Truth about Tobacco & the Power of Brief Intervention
This webinar will be presented by Lisa Fronzi, Clinical Nurse Specialist. She works in the Stroke Prevention Clinic at the Hamilton General Hospital.

- Describe the health impact of tobacco use
- Describe the basics of tobacco products, addiction and prevalence
- Describe the impact of providing brief intervention to all patients
- Describe your role in addressing tobacco use and nicotine addiction
- Apply the brief intervention model (the 3As) when counselling patients for tobacco cessation
- List and describe pharmacotherapy options for smoking cessation
